Some patients need to be cared for outside of the hospital setting at home or care facility. Clinically stable patients could benefit from comprehensive best-practice homecare provided by registered staff with relevant clinical expertise. Within the EcoCare program, we recognise that professional patient care and patient safety are non-negotiable. Our nursing staff has dedicated assistance with 24/7 ICU nurse support. Early clinical intervention reduces preventable re-admissions.

The clinician prescribed, patient specific care plans are implemented, monitored and quality assured through our CareMan® platform. Completing the care package with high quality equipment and full-time remote monitoring, EcoCare offers the ideal program for you patient needing advanced care at home.

The patient pathway

  1. Patient in Hospital
  2. Referral to EcoCare
  3. Assessment within 36 hours
  4. Submit Funder Application within 24hrs from Assessment
  5. In Hospital on Home Ventilator within 48hrs from Assessment
  6. Transition to Home within 3 to 5 days**
  7. Full-time nursing care for Authorised period
  8. Train Staff/Family during nursing care period
  9. Ongoing periodic patient visits by Registered nurse.

** Only when the patient is stable and treating physician approved discharge.
